In the course of providing physiotherapy services, we collect personal information that may include your name, contact details, date of birth, billing information and health information. Health information is considered sensitive information under Australian privacy law.
We only collect information that is necessary to provide high quality clinical care, manage appointments, process payments, meet legal and professional obligations, and support the safe and effective operation of our practice.
Information is collected directly from you wherever possible. In some circumstances, we may also receive information from referring doctors, other health practitioners, insurers, or third parties involved in your care.
Your information is primarily used to provide physiotherapy assessment and treatment. It may also be used for related purposes including:
communication with other health professionals involved in your care
practice administration and quality improvement
billing and processing of claims
compliance with legal, regulatory and professional requirements
This is We may disclose relevant information to your treating doctor, other health service providers, health insurers, third party insurers, and workers’ compensation authorities where required for claim assessment and payment.

From time to time, we may contact you with information about our services, practice updates, or physiotherapy related information that we believe may be of interest to you. This may include email or electronic communications.
You can opt out of receiving marketing communications at any time by contacting our administration team or using the unsubscribe option included in our emails. Opting out of marketing communications will not affect your access to clinical care.
We take reasonable steps to protect your personal and health information from misuse, interference, loss, unauthorised access, modification or disclosure.
Your information is stored securely in both electronic and paper-based systems. Electronic records are held within our cloud based practice management software, Nookal, which uses password protection, user access controls, encryption and secure servers.
Access to personal information is limited to authorised members of our team who require the information to provide care, manage appointments, process billing, or meet legal and professional obligations. All staff and contractors are bound by confidentiality and privacy obligations.
While we take data security seriously, no system is completely risk free. In the event of a data breach that is likely to result in serious harm, we will comply with our obligations under the Notifiable Data Breaches scheme. This includes taking steps to contain and assess the breach, and notifying affected individuals and the Office of the Australian Information Commissioner where required.
You have the right to request access to the personal information we hold about you and to request corrections if you believe the information is inaccurate, incomplete, or out of date.
Requests for access or correction should be made in writing. We may charge reasonable administrative costs for providing access, and additional time may be required to retrieve and prepare records. In some circumstances, access may be restricted as permitted under privacy legislation.
We aim to ensure that the information we hold about you is accurate, complete and up to date. To assist us, please notify our practice if your personal details change or if you believe any information we hold is incorrect.
If you have a concern about how your personal information has been handled, we encourage you to raise it with the Operations Manager. We will investigate all complaints fairly and promptly and aim to resolve concerns in a timely manner.
If you are not satisfied with our response, you may lodge a complaint with the Office of the Australian Information Commissioner.
