Active Connect is a structured telehealth post operative rehabilitation pathway delivered by Active Rehabilitation Physiotherapy. It is designed to support patients and health systems across the full surgical continuum.

The pathway spans pre operative preparation, inpatient admission, and post-operative recovery, providing continuity of rehabilitation beyond discharge and maintaining clinical oversight during the early recovery period.

Patients progress through structured rehabilitation with access to physiotherapy input as clinical needs arise, supported by clear governance, defined response timeframes, and multidisciplinary escalation processes.

In 2025, Active managed 1,893 lower limb arthroplasty cases and delivered more than 5,500 telehealth appointments, reflecting a mature and established telehealth model.

Active also supports Rehab in the Home patients from BPH via telehealth, providing continuity of care without in home visits and aligning with hospital-based care models.
