Active Connect replaces fragmented, appointment-driven follow-up with a governed rehabilitation pathway. Patients move through a defined programme with structured support, clear milestones, and access to their physiotherapy team as clinical needs arise.
Step 1: Pre-Operative Preparation
Before surgery, patients are engaged with education, expectation-setting, and baseline assessment. Starting rehabilitation before the operating table means patients arrive informed, prepared, and ready to recover.
Step 2: Structured Home Rehabilitation
Post-discharge, patients progress through a defined rehabilitation pathway with guided exercise programming, milestone-based progressions, and clear direction on what to focus on at each stage of recovery.
Step 3: Governed Access & Escalation
Patients can contact their physiotherapy team via secure messaging or telehealth review within agreed response timeframes. Concerns — whether relating to pain, mobility, wound status, or general progress — are reviewed promptly and escalated to the right clinical team when required.
